Lilly Earnings: Can Volume Keep Outrunning Price?

Analyst Targets

  • Goldman Sachs: Buy, $1,413 (raised October 21)
  • Bank of America: Buy, $1,397 (raised October 20)
  • RBC Capital: Buy, $1,500 (maintained October 12)

Eli Lilly reports third-quarter results before the open on Thursday, October 29, 2026. LLY is up about 19% year to date, hit a record intraday high of $1,311.75 on October 20, and sits well above Morningstar’s fair value estimate. At around 33 times forward earnings the stock trades below its five-year average of about 40 times, yet the premium to intrinsic value makes every quarterly data point load-bearing.

The Price vs. Volume Argument

Two quarters of filed data frame the problem. Revenue rose 56% to $19.8 billion in Q1 2026. Q2 added 48% more, reaching $23.0 billion on a 60% volume increase. Realized prices fell 13% globally, roughly 9% in the U.S. excluding rebate adjustments. Cash-pay Zepbound now starts at $299 per month on LillyDirect, and government agreements extended further discounts to eligible Medicare patients through the Medicare GLP-1 Bridge demonstration.

Volume has carried the income statement. Mounjaro hit $9.9 billion in Q2, up 91%. Zepbound reached $4.9 billion. Together they generated roughly two-thirds of quarterly revenue. Goldman estimates Zepbound will produce $5.3 billion in Q3, about 5% above consensus, supported by prescriptions running well above year-ago levels. Medicare’s GLP-1 Bridge demonstration launched July 1 and has already enrolled about 1.1 million patients, according to a recent industry report. The margin of safety on the volume-over-price trade narrows each quarter.

What Thursday’s Numbers Need to Show

Consensus revenue sits at $22.1 to $22.5 billion. Goldman models $23.1 billion. Full-year guidance from management is $85 to $87 billion; consensus is already at $88.6 billion. To satisfy holders at current prices, the guidance raise has to clear the Street, not Lilly’s own prior ceiling. Lilly’s latest update also reflected acquired in-process R&D charges tied to business development activity, which can create headline EPS noise that masks an operationally strong quarter.

The Medicare GLP-1 Bridge program is not the only policy clock running on weight-loss drugs. Telehealth platforms built around virtual GLP-1 prescribing face a DEA controlled-substance deadline that could close their prescription window at year-end.

Foundayo and What Comes Next

Orforglipron, branded Foundayo, received FDA approval for obesity on April 1, 2026, an oral GLP-1 for weight loss that can be taken any time of day without food or water restrictions. In a pivotal Phase 3 trial, participants at the highest dose lost 12.4% of body weight on average at 72 weeks. Lilly has also said it has submitted orforglipron for weight management and/or type 2 diabetes in more than 40 countries. Retatrutide remains in Phase 3 across obesity and obesity-related indications, and Lilly has said it plans to submit a biologics license application for retatrutide in Q1 2027. Goldman notes Foundayo expectations are in line with estimates: it is not moving Q3 numbers yet, but it is a primary reason the multiple holds.

Bull / Base / Bear

Bull: Revenue clears $23 billion, guidance rises above $89 billion, and Foundayo early scripts validate the oral GLP-1 category. Targets move toward $1,500.

Base: Revenue of $22 to $23 billion, guidance raised modestly to $87 to $89 billion. The stock consolidates between $1,250 and $1,350 heading into retatrutide’s BLA filing.

Bear: U.S. realized prices erode faster than volume can offset, Foundayo scripts disappoint, and guidance holds flat. LLY tests $1,150 to $1,200, near the 50-day moving average at $1,180.

Bottom Line

Thursday’s debate is not about whether Lilly beats. Two straight quarters have made a beat the default. The real question is whether the guidance raise clears consensus to justify a stock trading at a clear premium to Morningstar’s intrinsic estimate while realized GLP-1 prices slide. Volume has carried that case all year. Foundayo has to start contributing before the margin of error runs out.

Lilly is not the only large-cap biopharma where the market’s reaction to a data print has defied the underlying numbers. A stock can lose a quarter of its value on genuinely positive clinical results when the valuation already prices in perfection.
