Pfizer Reports Tuesday. The $10 Billion Obesity Bet Matters Most.

Analyst Targets

  • Guggenheim: Buy, $31.00 price target, models Q3 revenue at $16.16B and EPS of $0.77, above consensus
  • JPMorgan Chase: Neutral, $30.00 (raised from $28.00 in October)
  • BMO Capital Markets: Outperform, $30.00
  • Morgan Stanley: Equal-weight, $33.00
  • Bank of America: Neutral, $30.00
  • Street consensus: Hold, avg. price target ~$28.72

The Setup Into Tuesday

Pfizer drops its third-quarter results before the open on November 3. Consensus EPS sits around $0.72, on revenue near $15.9 billion to $16.1 billion depending on the aggregator. That compares to $0.87 and $16.65 billion a year ago, a year-over-year EPS decline of roughly 17%, revenue off about 4%. The trajectory is not the controversy. The Metsera pipeline is.

Full-year 2026 EPS estimates have been trimmed from $2.98 to $2.94 over the past 30 days, reflecting a 4.2% downward revision in the Q3 consensus over the same window. Pfizer’s guidance remains $2.80 to $3.00 adjusted EPS on revenue of $60.5 billion to $62.5 billion. Management has held that range through two straight quarters of beats: Q1 came in at $0.75 adjusted versus $0.72 expected, Q2 at $0.77 adjusted versus $0.68 expected.

Company Profile

Pfizer is a diversified global pharmaceutical company with primary revenue drivers across primary care (Prevnar, Eliquis), oncology (Padcev, Ibrance), and residual COVID products (Comirnaty, Paxlovid). The COVID franchise, which inflated earnings through 2022 and 2023, now shrinks the top line: management cut its full-year 2026 COVID product revenue forecast to roughly $4 billion, down from $5 billion at the start of 2026. Ex-COVID, the non-pandemic business grew 1% operationally in Q2. That number is the core story the bulls are counting on.

The Numbers to Watch Tuesday

  • Consensus EPS: $0.72 (Zacks/Yahoo) to $0.73 (TipRanks), prior year: $0.87
  • Consensus revenue: $15.86B (MarketBeat) to $16.13B (Zacks Visible Alpha)
  • Prevnar family: consensus at $1.66B, down ~5% year-over-year
  • Eliquis: Guggenheim models $2.44B vs. street at $2.32B
  • Ibrance: consensus near $1.03B
  • COVID products (Comirnaty + Paxlovid): street at $1.21B combined
  • Full-year guidance: Does management maintain $2.80-$3.00 EPS and $60.5B-$62.5B revenue?

Why Metsera Changes the Calculus

Pfizer paid roughly $10 billion for Metsera. The lead asset, berobenatide (a monthly injectable GLP-1 receptor agonist), produced up to 12.3% placebo-adjusted weight loss after 28 weeks in Phase 2b at monthly dosing. One Leerink Partners analyst called results “slightly inferior” to Eli Lilly’s Zepbound at a comparable trial point. Phase 3 trials are running at doses selected from Phase 2b, which is where the bull case lives.

The complications arrived fast. Pfizer’s August pipeline update axed two additional Metsera-sourced obesity programs, including an oral GLP-1 candidate. Berobenatide Phase 3 remains on track for a targeted 2028 launch, but every discontinuation raises the cost-per-asset math on a $10 billion check Pfizer wrote while carrying a forward multiple of roughly 10 to 11 times earnings.

Macro and Competitive Context

Eli Lilly and Novo Nordisk have effectively set the commercial bar in obesity at 20%-plus weight loss with proven cardiovascular data. Pfizer is years behind, launching into a market that will be considerably more crowded by 2028. The Most-Favored-Nation pricing policy and current tariff environment are both baked into guidance, management says, but the actual magnitude of those headwinds remains a live variable. Declining birth rates and shifting vaccine policy continue to pressure Prevnar, an asset that generated roughly $6 billion annually at peak.

The patent cliff pressure shaping Pfizer’s obesity bet is not unique to Pfizer. Big pharma’s scramble to replace expiring blockbusters has turned Chinese biotech into the industry’s primary early-stage pipeline source.

Forward Scenarios

Bull

Q3 comes in above $0.75, management raises the revenue guidance floor again, and Bourla provides early berobenatide Phase 3 dosing context showing improvement over Phase 2b. The stock re-rates toward the $33 to $36 range where the optimistic analyst cluster sits.

The bull scenario depends on Pfizer closing the gap with a market leader that is not standing still. Eli Lilly has been deploying its obesity cash flow into acquisitions and new therapeutic areas at a pace Pfizer cannot yet match.

Base

An in-line quarter near $0.72-$0.73 with guidance maintained and no meaningful obesity pipeline update. Stock stays range-bound near current levels around $29-$30.

Bear

A miss on Eliquis or Prevnar, guidance trimmed, and no constructive berobenatide signal. 2026 EPS estimates drift toward the bottom of guidance, and the forward multiple contracts further on a stock already showing an EPS decline year-over-year in Q3 terms.

Technical Overlay

PFE trades near $29.80, above the street consensus target of $28.72. The stock’s forward P/E near 10 to 11 times represents a significant discount to the broader large-cap drugmaker group, which screens closer to the high-teens on forward earnings. Recent quarterly beats provide some near-term support, but gap-downs on guidance cuts have been swift in this name across 2024 and 2025.

Bottom Line

The Q3 number itself is secondary. What investors are actually pricing on November 3 is whether Pfizer’s management can articulate a credible 2028 obesity revenue bridge while managing a patent cliff that Eliquis will eventually steepen. Two straight beats bought goodwill. The third needs to come with a more detailed berobenatide Phase 3 readout to move the stock in any meaningful direction.
